Application Process for Market Stall

New Applicant

  1. Application Form
  2. Residential Sketch
  3. Cedula (current year)
  4. Latest 2×2 current picture w/ name tag
  5. Birth Certificate
  6. Marriage Contract (for married)

Transfer of Stall

  1. Application Form
  2. Residential Sketch
  3. Cedula (current year)
  4. Lates 2×2 current picture w/ name tag
  5. Birth Certificate (if transfer from father or mother to his/her child)
  6. Death Certificate (in case of death of stallholder)
  7. Marriage Contract (if transfer is from husband to wife or vise versa)
  8. Affidavit of no objection (from siblings, husband,/wife, mother of father)
  9. Affidavit of Waiver of Rights

Issuance of Lease Contract

For Renewal

  1. Business Permit (preceding year)
  2. Lease Contract (preceding year)
  3. Cedula (current year)
  4. Latest 2×2 colored picture w/ name tag
  5. A.L.I.A.S (proof of payment) of stall rentals, etc.)
  6. Official Receipt (OR)(certification for adjacent stall)
  7. Billing Statement with corresponding OR (proof of payment)
  8. Sanitary Permit
  9. Declogging Permit (CR)

For New Awardee

  1. Certificate of Award
  2. Cedula (current year)
  3. Latest 2×2 colored picture w/ name tag
  4. OR (for adjacent stall)
